Before filling out a registration form, check you are in our catchment area.
You can also find out which GPs you can register with on the NHS website.
Registering with the surgery
To register with the surgery:
- fill out a new patient registration form online
- use the register with a GP surgery service
- fill out a temporary patients registration form online
When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
Registration check
You will need to request an appointment to see Dr Rogers for a registration check.
Please bring a urine sample with you (bottles available from reception).
Temporary residents
Our doctor is happy to see patients who are temporarily resident in the practice catchment area should medical advice be required. In this instance, persons wishing to be treated should register as a temporary patient.
