Travel risk assessment

Use this service if you are travelling abroad and need travel-health advice or vaccinations.

This gives the clinical team time to assess your needs and, where appropriate, order and administer vaccinations before your departure.

Please complete a separate form for each person travelling.

If you are travelling in less than 8 weeks, submit the form as soon as possible. We may not have enough time to provide every recommended vaccination before you leave, but the clinical team can still review your information and advise you about your options.

Who can use this service?

You can use this service if you are registered with Fleggburgh Surgery. You can also complete the form for another registered patient.

Before you start

It will help to have the following information available:

• your destinations and travel dates
• the type of accommodation and activities planned
• details of previous travel vaccinations, if known
• your current medication and relevant medical conditions
• your contact details
• your details, if completing the form for someone else

What happens next?

After you submit the form, a member of the clinical team will review your travel plans and vaccination history.

We will contact you with advice and let you know whether an appointment is needed. Submitting the form does not automatically book an appointment.

Some travel vaccinations and services are not available through the NHS and may involve a charge. We will explain any relevant options after reviewing your assessment.

If you cannot complete the online form because of a disability or another individual circumstance, please contact the surgery so we can help.
